The undersigned hereby releases from liability (business)___________________________ and its manager, _________________________________________________________
at (location and phone #)___________________________________________________
for works of art provided by the undersigned for display at the above site. The undersigned holds the above parties not responsible for the mounting, sale or maintenance of their works of art for the duration of the time during which these works of art are displayed at this location and until these works of art are removed from said premises by their owner. The undersigned also promises to abide by the rules as stated by the manager regarding the mounting and display of said art on establishment premises. 

It is further understood, that ownership of any and all works of art as listed herein remains with the artist, and that the art may be removed at any time by the owning artist upon notification to the manager.
